What Is Mounjaro and How Does It Work?

Mounjaro contains tirzepatide, the first medicine of its kind to act on two natural gut hormone receptors at once: GLP-1 (glucagon-like peptide-1) and GIP (glucose-dependent insulinotropic polypeptide). Both hormones are normally released after eating and play a key role in appetite, fullness and how the body handles sugar and fat.

According to its Summary of Product Characteristics (SPC), tirzepatide works by reducing appetite and food intake, slowing the rate at which the stomach empties so you feel fuller for longer, and improving the way the body responds to insulin. Together these effects help you eat less without the constant hunger that makes traditional dieting so difficult to sustain.

Because it targets two receptors rather than one, many patients find Mounjaro delivers greater weight loss than single-hormone (GLP-1 only) treatments — which is reflected in the clinical trial results below.

    • Dual GIP and GLP-1 receptor agonist — a different mechanism to Wegovy
    • Reduces appetite and cravings, and increases feelings of fullness
    • Slows stomach emptying so meals satisfy you for longer
    • Supports improvements in blood sugar, blood pressure and cholesterol seen in trials

The Research and Evidence Behind Mounjaro

Mounjaro's licence for weight management is based on the SURMOUNT clinical trial programme, involving thousands of adults living with obesity or overweight. In the landmark SURMOUNT-1 study, published in the New England Journal of Medicine, adults taking the highest dose of tirzepatide alongside lifestyle changes lost an average of around 21% of their body weight over 72 weeks, compared with around 3% with placebo.

To put that in context: for someone weighing 16 stone (about 100kg), that is an average loss of over 3 stone. More than half of participants on the higher doses lost 20% or more of their body weight. Most people notice steady progress early on — a typical, healthy expectation is around half a stone in the first couple of months as the dose builds up, with weight loss continuing as you reach your maintenance dose.

Weight loss with Mounjaro is gradual and sustained rather than a quick fix, and the best results in every trial were achieved when the medicine was combined with dietary changes and increased activity — exactly the support our coaching provides.

Doses and How Treatment Is Built Up

Mounjaro is started at a low dose and increased gradually, usually every 4 weeks, to give your body time to adjust and to minimise side effects. Your prescribing pharmacist will guide your dose at every step — you never increase without a review.

    Dose / StrengthTypically usedNotes
2.5mg weeklyWeeks 1–4Starting dose to build tolerance
    5mg weeklyWeeks 5–8First maintenance step
7.5mg weeklyFrom week 9Increased if needed and tolerated
10mg weeklyOngoingIncreased if needed and tolerated
12.5mg weeklyOngoingIncreased if needed and tolerated
    15mg weeklyOngoingMaximum dose

Doses are only increased after review with our pharmacist, and many patients achieve excellent results without reaching the maximum dose. The injection is given under the skin of the stomach, thigh or upper arm using a simple pre-filled KwikPen — we teach you exactly how at your first appointment.

    Side Effects and Safety

Like all medicines, Mounjaro can cause side effects, although not everybody gets them. Most are digestive, mild to moderate, and settle as your body adjusts — they are most noticeable when starting treatment or moving up a dose. The gradual dose build-up is specifically designed to minimise them.

Common side effects (usually mild and temporary)

    • Nausea (feeling sick) — the most common, usually easing within days to weeks
    • Diarrhoea or constipation
    • Vomiting or indigestion
    • Reduced appetite (part of how the medicine works)
    • Stomach (abdominal) pain, bloating, burping or wind
    • Tiredness, dizziness or headache
    • Injection site reactions such as redness or itching
    • Hair thinning has been reported in some patients during rapid weight loss

Less common but important — seek advice promptly

    • Severe, persistent stomach pain (with or without vomiting) — possible pancreatitis; stop treatment and seek urgent medical help
    • Gallstones or gallbladder problems — pain in the upper right stomach, fever or yellowing of the skin/eyes
    • Severe dehydration from prolonged vomiting or diarrhoea, which can affect the kidneys — keep drinking fluids
    • Low blood sugar (mainly if you also take certain diabetes medicines such as insulin or sulfonylureas)
    • Signs of a serious allergic reaction — swelling of the face, lips or throat, difficulty breathing
    • Mounjaro must not be used in pregnancy — additional barrier contraception is advised for women on oral contraceptives for 4 weeks after starting and after each dose increase

This is a summary, not a complete list — full details are in the patient information leaflet and the Mounjaro Summary of Product Characteristics, which we review with you. Our team is available throughout your treatment to manage any side effects, adjust your dose or slow your titration where needed.

How Our Prescribing Pharmacist Keeps You Safe

Mounjaro is a prescription-only medicine, and using it safely takes more than a prescription. As an independent prescribing pharmacist-led clinic, we build clinical checks and real human support into every stage of your treatment — face-to-face, at the pharmacy, whenever you need us.

    Full suitability assessment

Before anything is prescribed we review your BMI, medical history, current medicines and any conditions that make treatment unsuitable — such as pregnancy, a history of pancreatitis or certain thyroid conditions.

    Safe dose titration

We follow the licensed dose escalation schedule and only increase your dose after reviewing your progress and side effects, keeping you at each step longer if your body needs it.

    Injection training

At your first appointment we show you exactly how to use the pen, where to inject, how to rotate sites and how to store your medicine correctly.

    Ongoing monitoring

Regular follow-ups track your weight, side effects, blood pressure and overall response, so treatment is adjusted early rather than left to drift.

    Side effect management

Practical advice on eating patterns, hydration and dose timing that dramatically reduces nausea and digestive symptoms — and a direct line to a clinician if anything concerns you.

    Interaction and safety checks

We check every medicine you take — including contraception, diabetes treatment and anything newly started — and liaise with your GP where appropriate.
