Both are released naturally after you eat and together regulate appetite, fullness and how your body handles sugar and fat.

The practical effect, as described in its Summary of Product Characteristics, is a powerful reduction in appetite and food intake, slower stomach emptying so you stay full for longer, and improved insulin response. Patients typically describe portion sizes shrinking naturally and the mental battle with cravings quietening down.

The trial results that made headlines

In the landmark SURMOUNT-1 trial, published in the New England Journal of Medicine, adults taking the highest dose of tirzepatide alongside lifestyle changes lost an average of around 21% of their body weight over 72 weeks — the strongest result of any licensed weight loss medicine. More than half of participants on higher doses lost a fifth of their body weight or more.

What does that mean for a typical patient? For someone starting at 17 stone, a 21% average is over 3.5 stone. But it's important to set expectations correctly: weight loss is gradual by design. The dose starts low at 2.5mg weekly and builds up every four weeks, so in the first couple of months a realistic expectation is around half a stone — with the bigger losses accumulating steadily over the following year as you reach your effective dose.

Trials also recorded meaningful improvements in blood pressure, cholesterol and blood sugar — which is why we think of this as a health treatment that shows up on the scales, not just a slimming aid.

The side effects nobody should gloss over

Mounjaro is well tolerated by most people, but it's a serious medicine and deserves honest discussion. The most common side effects are digestive — nausea, diarrhoea or constipation, vomiting and reduced appetite — typically worst in the first weeks or after a dose increase, then settling. Practical adjustments (smaller meals, less fatty food, good hydration, dose timing) make a big difference, and this is where having a clinician you can actually speak to earns its keep.

Rarer but important risks include pancreatitis (severe persistent stomach pain needs urgent attention), gallbladder problems, and dehydration if vomiting is prolonged. Mounjaro can't be used in pregnancy, and women using oral contraception need additional barrier contraception for four weeks after starting and after each dose increase — a detail some online providers never mention, and one we always cover.
